About the Role

The registered nurse plans, organizes, and directs hospice care, emphasizing community health education and experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.

Responsibilities

  • Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a full physical assessment and history of current and previous illnesses.
  • Provides professional nursing care using all elements of the nursing process.
  • Assesses and evaluates patient status by writing and initiating the plan of care, regularly re-evaluating patient and family needs, and revising the plan as necessary.
  • Uses health assessment data to determine nursing diagnoses and develops a care plan with goals based on these diagnoses, incorporating palliative nursing actions.
  • Includes the patient and family in the planning process and initiates preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician in the plan of care.
  • Counsels patients and families on meeting nursing and related needs, providing health care instructions as appropriate.
  • Assists patients with activities of daily living and facilitates efforts toward self-sufficiency and comfort care.
  • Acts as Case Manager when assigned, coordinating patient care for an assigned caseload.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and care, including pain/symptom management changes.
  • Communicates with physicians regarding patient needs, reports changes in condition, and obtains physician orders as required.
  • Coordinates with community health-related persons to ensure the care plan is followed.
  • Reports failures to comply with Florida Department of Health requirements within 14 days, as well as suspected abandonment, abuse, financial exploitation, or neglect to the appropriate agencies.
  • Teaches patients and families self-care techniques and provides health counseling.
  • Works with the interdisciplinary group to provide continuity of care and meet emotional needs of patients and families.
  • Attends interdisciplinary group meetings and ensures a safe environment for the patient.
  • Participates in on-call duties as defined by policy, including availability every other weekend.
  • Ensures arrangements for necessary equipment and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Maintains and upgrades professional knowledge through continuing education and inservice classes.
  • Fulfills case assignments and actively participates in quality assessment and performance improvement activities.

Qualifications

  • Graduate of an accredited school of nursing.
  • Registered Nurse with a valid Florida license in good standing.
  • Minimum of one (1) year of professional nursing experience within the last three (3) years or a baccalaureate degree in nursing with at least two (2) years of experience, preferably in public health, home care, or hospice nursing.
  • Responsible for supervising hospice aides.
  • Satisfactory references and a clear criminal background check (required post-offer).

Skills

  • Excellent observation, verbal, and written communication skills.
  • Strong problem-solving and basic math skills.
  • Proficient in nursing skills per competency checklist.
  • Ability to perform prolonged standing, walking, lifting, positioning, and transferring patients.
  • Capable of reaching, stooping, bending, kneeling, and crouching.
  • Visual acuity and hearing sufficient to perform required nursing skills.
  • Valid driver’s license with an insured and reliable automobile in good working order.
  • Current CPR certification (must include hands-on skills demonstration).
