About the role

A highly respected Senior Living Community is currently hiring a full-time, permanent experienced, Hospice and Palliative Care Registered Nurse (RN) to join their clinical nursing team. $2,000 Sign-On Bonus!

Responsibilities

• Coordinating and managing hospice plans of care
• Conducting comprehensive patient assessments
• Providing compassionate nursing care and symptom management
• Collaborating with physicians, social workers, chaplains, and other team members
• Educating patients and families regarding disease progression and care expectations
• Serving as a patient advocate and resource for end-of-life care
• Ensuring timely and accurate documentation

Qualifications

• Active Delaware Registered Nurse (RN) license required
• Minimum 2 - 3 years of hospice/palliative care nursing experience strongly preferred
• Candidates with significant experience in home health, geriatrics, oncology, or other closely related clinical settings may be considered
• Current CPR certification required
• Hospice and Palliative Nursing Certification (CHPN) preferred
• Strong clinical assessment and communication skills
