About the Role
Provide home-based care to patients with six months or less to live, focused on quality of life. Work as part of an interdisciplinary team—including a physician, LPN, CNA, HHA, social worker, chaplain, and volunteers—to carry out the patient’s plan of care. Take a holistic approach that engages the patient’s mind, body, spirit, and family.
Responsibilities
- Perform patient assessments, check vitals, evaluate breathing, and monitor pain
- Administer prescribed medication and treatments
- Ensure accessibility of medical equipment, prescribed oxygen, medication, and other supplies
- Educate family on the patient’s prognosis, changes in care plan, medication, and hospice goals
Requirements
- Graduate of an accredited nursing program and active RN license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills, and compassion
