About the role

The Registered Nurse (RN) Case Manager provides care and services for a group of patients, visiting them where they reside—including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources.

• Work closely with patients on a 1:1 level while receiving strong support from a patient care team.
• Operate independently on a day-to-day basis, handling phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
• Manage visits, documentation, and meetings with strong time management skills, prioritizing and triaging multiple concerns.
• Learn and use an electronic medical records system.
• Drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.

Pay

$90,000–$95,000 (commensurate with experience).

Schedule

Full-time, Monday–Friday, daytime hours.
