About the role

The Registered Nurse (RN) at Suncrest Hospice will provide care and services for a group of patients, visiting them in their homes, Skilled Nursing Facilities, and assisted living facilities. This role is a contributing member of the interdisciplinary team, focusing on psychosocial assessments, interventions, and knowledge of community resources.

Responsibilities

  • Provide care and services for a group of patients, visiting them in their homes, Skilled Nursing Facilities, and assisted living facilities.
  • Collaborate with the interdisciplinary team, providing expertise in psychosocial assessments, interventions, and knowledge of community sources.
  • Work independently and on your own, managing activities such as phone, email, and written communication, attending meetings with various healthcare professionals, and understanding hospice rules and regulations.
  • Manage visits, documentation, and meetings through time management skills, prioritizing and triaging multiple concerns.
  • Learn and utilize an electronic medical records system.

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio.
  • Works well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and aide leadership.
  • Can work independently and on your own, managing activities including phone, email, and written communication, attending meetings with a variety of healthcare professionals, and understanding hospice rules and regulations.
  • Must have the ability to drive with reliable transportation, a valid driver’s license, and auto insurance, to travel across the service area.

  • Strong clinical skills and experience in hospice care.
  • Excellent communication and interpersonal skills.
  • Ability to handle sensitive and confidential information.
