About the Role
The Hospice Registered Nurse provides and documents skilled nursing care according to care plans and physician’s orders, ensuring confidentiality and professional boundaries.
Responsibilities
- Deliver and coordinate quality patient care in line with physician instructions.
- Assess patient conditions continuously, reporting changes promptly.
- Document assessments, actions, responses, and care coordination accurately.
- Develop, implement, and update nursing care plans.
- Take appropriate nursing actions to meet expected outcomes.
- Recognize and respond to changes in patient needs to prevent or reduce risks.
- Ensure compliance with policies, regulations, and the Nurse Practice Act.
- Practice infection control measures per OSHA standards.
- Educate patients and families on disease management, self-care, and prevention.
- Provide compassionate, compliant nursing care in patients’ homes.
- Assess and respond to patient needs promptly.
- Educate patients and families on health management.
- Maintain professional growth through continued education.
- Adhere to infection control and safety protocols.
Requirements
- Graduate of an accredited Nursing program (Diploma, Associate, or Baccalaureate).
- Current, unrestricted RN license in the practice state.
- Current CPR certification.
- Valid driver’s license with auto insurance and acceptable MVR.
