Hospice Registered Nurse Case Manager Full Time

About the Role

The Hospice Registered Nurse provides skilled nursing care following care plans and physician's orders, ensuring confidentiality and professional standards.

Responsibilities

• Deliver and coordinate quality patient care in accordance with physician instructions.
• Assess patient conditions continuously, report changes, and document all activities promptly.
• Develop, implement, and update nursing care plans.
• Take appropriate nursing actions based on assessments to achieve desired outcomes.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Ensure compliance with policies, regulations, and the Nurse Practice Act.
• Use infection control measures per OSHA standards.
• Educate patients and families on disease management, self-care, and prevention.
• Maintain professional competencies through ongoing education and skill evaluations.

Requirements

• Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
• Current, unrestricted RN license in the state of practice.
• CPR certification.
• Valid driver’s license with auto insurance and acceptable MVR.

Schedule

The role involves working Monday to Friday with an on-call rotation across multiple counties.
