Hospice Registered Nurse Case Manager

About the role

Providence at Home with Compassus seeks a Hospice Registered Nurse (RN) to deliver person‑centered hospice and palliative care in patients’ homes. The RN assesses patient and family needs, develops and implements individualized plans of care in collaboration with the Interdisciplinary Group (IDG), provides nursing care using the full nursing process, administers medications, participates in on‑call duties, and maintains documentation while supervising ancillary staff and pursuing continuing education.

Responsibilities

• Complete patient and family assessments to determine hospice needs.
• Perform comprehensive physical assessments and histories; provide professional nursing care using all elements of the nursing process.
• Write and initiate the Plan of Care in collaboration with the IDG; regularly reevaluate and revise as needed.
• Develop care plans with patient, family, caregiver, and IDG that establish goals based on nursing diagnosis and incorporate palliative nursing actions.
• Administer medications and treatments as prescribed by the IDG‑approved Plan of Care.
• Participate in on‑call duties per organizational procedures.
• Maintain productivity standards set by organizational guidelines.
• Complete, maintain, and submit accurate clinical notes; supervise ancillary personnel and delegate responsibilities.
• Participate in orientation, education, and development of team members.
• Pursue personal growth through continuing education and in‑service classes.

Requirements

• Associate or Bachelor’s degree from an accredited School of Nursing.
• Current, unencumbered RN license in the state of employment.
• Preferred: 1 year of RN experience; hospice CNA or LPN experience considered.
• Required within 30 days of hire: National Provider BLS – American Heart Association (vendor‑managed).

Pay

Compensation range: $45.73 – $70.00 per hour (base pay; additional compensation possible for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities).
