Hospice Registered Nurse Case Manager

Responsibilities

• Coordinate and deliver end-of-life care in patients’ homes and facilities.
• Manage a caseload of hospice patients, conducting comprehensive assessments.
• Develop and update individualized care plans tailored to patient needs.
• Provide skilled nursing, pain and symptom management, and patient/family education.
• Collaborate with physicians, social workers, chaplains, and aides to ensure coordinated care.
• Ensure timely visits, accurate documentation, and compliance with regulations.
• Support families throughout the hospice journey.

Qualifications & Skills

• Registered Nurse (RN) license.
• Experience in hospice or palliative care preferred.
• Strong assessment and care planning abilities.
• Excellent communication and collaboration skills.
• Knowledge of regulatory requirements and documentation standards.
