About the Role
Provide compassionate end-of-life care in patients’ homes as a Hospice Registered Nurse Case Manager. You will coordinate and deliver skilled nursing care, manage pain and symptoms, and work closely with an interdisciplinary team to ensure high-quality, individualized care.
Responsibilities
- Coordinate and deliver skilled nursing care in patients’ homes
- Manage pain and symptoms effectively
- Develop individualized care plans in collaboration with an interdisciplinary team
- Educate and support patients and families
- Ensure timely and accurate documentation
- Communicate closely with physicians and caregivers
Requirements
- Registered Nurse licensure
- Strong communication and interpersonal skills
- Ability to work independently with autonomy
