Hospice - PRN - Nurse Practitioner (NP)

About the Role

This is a per-visit role where the Nurse Practitioner assists in overseeing the clinical care and treatment of patients and families, consults with attending physicians, and provides clinical direction to the Interdisciplinary Group/Team to ensure quality care.

Responsibilities

• Collaborates with the Interdisciplinary Group/Team (IDG/T) to ensure appropriate evaluation and palliative treatment of terminal disease.
• Consults with and assists the IDG/T in developing the patient/family plan of care to assure quality care is provided.
• Consults with and educates the patient/family regarding disease process, self-care techniques, and end-of-life care.
• Provides training to other staff as needed.
• Performs and documents visits to patient/family based on the plan of care and patient/family need.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
• Consults with and provides education to other IDG/T members on a PRN basis for clinical issues relating to palliation.
• Actively participates in the IDG/T.
• Communicates effectively, timely, and appropriately at all times.
• Informs physicians and other personnel of changes in the patient’s needs and outcomes of intervention while evaluating patient/family response to care.
• Completes face-to-face encounters with patients, as assigned, prior to the 3rd benefit period and each period afterward.
• Provides clinical assessment results to the hospice physician after making face-to-face encounters with patients, as assigned.
• Always uses standard precautions and follows hospice policy.
• Adheres to hospice policies regarding protected health information.

Qualifications

• Graduate from a Baccalaureate School of Nursing, a Nurse Practitioner (NP) focused Master’s or Doctoral Nursing Program.
• Current state license as a Registered Nurse and as an Advanced Practice Nurse.
• Two years’ experience as an Advanced Practice Nurse, clinical care setting preferred; hospice or home health experience preferred.
• Nursing skills as defined by generally accepted standards of practice.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.
• Good interpersonal skills.
• Reliable transportation, valid auto liability insurance, and valid driver's license.
