Hospice Patient Care Nurse (RN)

Role: Patient Care Manager, RN Responsibilities Lead and coordinate interdisciplinary hospice services for patients with life-limiting illnesses.Oversee clinical care planning and conduct patient assessments.Manage caseloads, ensuring high-quality pain and symptom management in home and facility settings.Supervise and mentor nursing staff.Collaborate with physicians, social workers, chaplains, and therapists.Support families through education and emotional guidance.
