Responsibilities
- Deliver patient care, including personal care (ADL) such as bathing, body mechanics, nutrition, elimination, safety, special procedures, and housekeeping services.
- Participate in interdisciplinary team conferences (IDT) and special case conferences.
- Assume responsibility for establishing and maintaining effective working relationships with team members.
- Identify specific patient/caregiver needs and make referrals to appropriate team members.
- Ensure accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure/certification standards.
- Demonstrate competency in knowledge and skills, maintain HPCCR professional reputation, and stay updated on healthcare trends.
- Clean patient rooms and ensure facility cleanliness, using proper chemicals and protective equipment.
Requirements
- High school diploma preferred, plus training/certification in related field required.
- Certified Nursing Assistant (CNA) license required.
- Must be in good standing with NC Department of Health and Human Services.
- Minimum 1 year experience required, preferably in home care, working with terminally ill, and/or geriatrics.

- Experience with various medical devices and treatments.

Salary is commensurate with experience.
