About the role
Care Initiatives is currently seeking a PRN Visit Nurse to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.
Responsibilities
- Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient.
- Provide consistent, high-quality patient care to ensure an exceptional and seamless patient experience.
- Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
- Provide high-quality patient care while ensuring clear, concise, and accurate clinical documentation.
- Assess the need for plan of care updates while keeping the patient and family involved in care planning decisions.
Requirements
- RN with a minimum of two years of recent clinical nursing experience.
- Current, valid CPR certification.
- Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support.
- Willingness and ability to provide care in facility and patient home settings.
