Hospice Registered Nurse (PRN)
About the role
The Registered Nurse (RN) at Suncrest Hospice will provide care and services to a group of patients, including those residing in private homes, Skilled Nursing Facilities, and assisted living facilities. This role involves working in collaboration with an interdisciplinary team, conducting psychosocial assessments, interventions, and utilizing knowledge of community resources.
Responsibilities
- Provide care and services to a group of patients, including those residing in private homes, Skilled Nursing Facilities, and assisted living facilities.
- Collaborate with an interdisciplinary team to conduct psychosocial assessments, interventions, and utilize knowledge of community resources.
- Work in one of three roles: Case Management, Admissions, or Resource.
- Work independently and on your own from a day-to-day basis with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Manage visits, documentation, meetings, and prioritize and triage multiple concerns.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Can work independently and on your own from a day-to-day basis with activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area.

Pay
Competitive Pay for PRN RN's: $35 - $41 BOE.
Schedule
Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns).
