Hospice Nurse

About the role

This position involves providing specialized nursing care and support to patients in a hospice setting, including assessment, care planning, and collaboration with an interprofessional team. When designated as a case manager, the nurse coordinates all aspects of patient care to ensure quality and continuity.

Responsibilities

• Assess patient preferences, cultural, spiritual, and emotional needs to tailor care plans.
• Develop, implement, and document individualized care plans with clear goals, ensuring effective communication across care teams.
• Promote respectful and empathetic interactions with diverse populations, supporting emotional and spiritual needs.
• Evaluate patient condition changes, update care plans, and communicate with families and team members.
• Ensure evidence-based nursing practices to achieve optimal patient outcomes.

Qualifications

• Nursing Diploma/Associate's Degree or higher, with RN licensure and a valid driver's license.
• Minimum of 2 years of nursing experience; preferred Bachelor's degree in Nursing or Home Health with 2–5 years of experience.
• Prior experience supporting hospice or palliative care is preferred.
• Ability to work safely and maintain alertness in a fast-paced environment.

Schedule

This role requires night shifts, weekend and holiday rotations.
