Hospice Nurse

About the Role

A Full-Time Hospice Registered Nurse at Luther Manor is responsible for fulfilling patients' total needs, including assessment, medication management, documentation, supervision of other nursing personnel, and acting as the Care Coordinator.

Responsibilities

• Participate as part of the core team providing care and services, coordinating the implementation of the plan of care.
• Participate in developing and revising written patient care policies and procedures.
• Participate in the development, review, and updating of a patient’s plan of care.
• Actively participate in the hospice’s quality assurance program.
• Assess patient and family needs as directed by the care coordinator.
• Promptly notify the Care Coordinator or Hospice Director of any change in patient status that suggests a need to update the plan of care.
• Provide services consistent with the patient plan of care.
• Provide education and counseling to the patient and family, consistent with the plan of care.
• Regularly assess the patient’s nursing needs, implement the plan of care provisions, and re-evaluate those needs.
• Ensure nursing services are available twenty-four (24) hours a day as part of the hospice program.
• Supervise and teach other nursing personnel, including licensed practical nurses, home health aides, and hospice aides, including their provision of care and documentation.
• Evaluate the effectiveness of delegated acts performed under the registered nurse’s supervision.
• Direct and staff nursing services to assure that the nursing needs of patients are met.
• Specify patient care responsibilities of nursing personnel.
• Provide services in accordance with recognized standards of practice.
• Maintain current knowledge of medications, administration, and monitoring.
• Perform ongoing observations, assessments, and documentation of patient and family needs.
• Notify the physician of changes in condition or needs.
• Communicate status to patient and family as needed.
• Complete all required documentation.
• Maintain honest, open, and professional communication with others.
• Provide consistent and fair disciplinary action when appropriate.
• Practice universal precautions.
• Demonstrate knowledge and skill in the use of medical equipment (e.g., suction, oxygen, nebulizer).
• Respond to patient/family calls within fifteen (15) minutes.
• Participate in continuing education opportunities.
• Abide by all Luther Manor Hospice Care Policies and Procedures as outlined in the employee handbook.
• Respect the right of the resident to select and refuse services.
• Perform other duties as assigned.

Requirements

• Current Wisconsin RN licensure.
• Mathematical ability to determine medication and fluid calculations.
• Ability to speak, hear, and write legibly to communicate with patients, families, and staff.
• Ability to organize tasks and function independently.
• Ability to maintain a good attendance record.
• Good personal hygiene.
• Physical ability to stand, walk, push, pull, bend, twist, sit, kneel, crouch, and reach high or low levels.
• Ability to use arms and hands to push, pull, grasp, or write.
• Physical strength to move or assist residents as needed following their plan of care; must be able to push, pull, or lift 50 pounds.
• Ability to tolerate exposure to blood and body fluids.
• Tolerant of hands being exposed to water, soap, and protective gloves.
• Tactful, empathetic, and courteous.
• Free of police record related to abuse or misappropriation of property.
• Valid Wisconsin driver’s license and a good driving record.
• Auto insurance with Luther Manor minimum coverage requirements.
