Hospice LPN - South Facilities Team

About the role

Under the direction of the RN case manager, the LPN provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in the plan of care, reporting significant changes in patient condition and family needs as needed. In collaboration with the RN, the LPN accepts and processes physician orders to maximize patient comfort.

Primary Tasks, Duties, And Responsibilities

• Provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in the plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.

Requirements

• Graduate of an approved nursing program
• Owns a current Florida LPN license
• Minimum one year experience in an acute care setting, providing direct patient care
• Previous experience as a hospice LPN preferred
• Completion of an approved 30 hour IV certification course required within 6 months of hire
