Salary: $25 - $33 / hour
Schedule: 8am - 5pm Monday - Friday

Be the Best Hospice LVN/LPN You Can Be
- Must have a current Licensed Practical Nurse (LPN) license in the state of practice; associate degree preferred.
- CPR certification required in Georgia and preferred elsewhere.
- Must meet all state and regulatory requirements where care is provided. One year of experience in a licensed healthcare setting is required (preferred in California), ideally in hospice, home health, geriatrics, or medical-surgical care.
- Must have a valid driver’s license, reliable vehicle, and state-required liability auto insurance.
- Must be committed to the organization’s values, mission, and philosophy of care, serving as a role model for team members.
- Physical demands include frequent walking, sitting, stooping, driving, typing, and lifting or moving up to 50 lbs; must assist with patient handling up to 200 lbs with help.
- Must be comfortable working in a variety of conditions, including occasional exposure to outdoor weather, noise, fumes, chemicals, and uneven surfaces.
Responsibilities
- Provide direct nursing care under the guidance of a Hospice RN to support patient comfort, health, and dignity across pre-hospice, continuous care, and in-home respite services.
- Collaborate with the interdisciplinary team (IDG), patients, families, and physicians to assess needs, coordinate care, and support holistic well-being.
- Maintain timely, accurate documentation and communicate any changes or concerns to the clinical team to ensure responsive, high-quality care.
- Educate patients and caregivers in alignment with the plan of care, promoting understanding, confidence, and comfort throughout the care journey.
- Participate in on-call rotations, scheduled visits, IDG meetings, and family conferences, contributing to seamless care coordination.
