Job Summary
Aid in coordinating the transition of patients from home health services to Palliative or Hospice, ensuring continuous medical care. Coordinate transfers from skilled nursing facilities (SNFs) or hospitals to Palliative or Hospice services, maintaining uninterrupted medical care. Provide patient care, utilizing various skills and necessary coordination of services. Ensure accurate and timely documentation according to policy and procedure and regulatory requirements. Assist in managing daily branch office tasks, including patient care documentation and supply management. Support the Hospice Registered Nurse with medication reconciliation and pharmacist recommendations.
Job Requirements
- Current LPN licensure within the state of branch location and at least two years of clinical nursing experience, preferably in a Home Health/Hospice setting
- Successfully completed nursing education from an approved school of nursing
- Ability to independently organize work and deliver effective healthcare services to hospice patients
- Ensure appropriate patient care as supervised by the Hospice Registered Nurse
- Document care thoroughly, accurately, and completely on a timely basis
- Accept travel and working conditions as assigned
- CPR certification
- Minimum of twelve hours of continuing education in a twelve-month period
