Requirements
- Licensed Practical Nursing Degree
- CPR/BLS Certified
- A minimum of 1 year of Clinical experience
- Hospice Experience Preferred
- Valid Drivers License and Auto insurance Coverage
- Reliable transportation
About the role
- Provide skilled nursing care under the supervision of an RN
- Assist Registered nurses in carrying out the plan of care
- Assist RN in performing specialized procedures (as defined in the state nurse practice act) as directed by the RN under physician's orders
- Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed
- Assist reinforcement of patient and family education regarding end-of-life care
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning
- Keep the RNCM informed of changes noted during the process
- Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading
