Hospice Liaison

You shouldn't have to apologize to referral sources for the hospice you represent. Hospice Care Consultant (HCC) role — company car included. Uncapped commission with top performers earning $200K+. This role is for the experienced hospice sales rep, the healthcare account executive ready to grow, or the clinician who's realized they'd be great at this. Responsibilities

• Build and maintain relationships with referral sources and healthcare professionals
• Represent Moments Hospice in a professional manner, emphasizing our unique approach to hospice care
• Identify and pursue new referral opportunities, ensuring compliance with all regulatory requirements
• Collaborate with clinical and operations teams to provide comprehensive care plans for patients and families
• Manage and grow your assigned territory, contributing to the overall growth of Moments Hospice
• Ensure timely and accurate documentation of all interactions and referrals

Requirements

• Experience in healthcare or hospice sales, or a relevant background (e.g., medical device, pharma, clinical liaison)
• Strong communication and interpersonal skills
• Ability to build and maintain relationships with healthcare professionals and referral sources
• Passion for hospice care and a commitment to providing exceptional patient and family care
• Self-motivated and able to work independently, as well as part of a team

Qualifications

• High school diploma or equivalent
• Some college or vocational training in healthcare or related field preferred
• Valid driver’s license and good driving record

Skills

• Excellent communication and interpersonal skills
• Ability to build and maintain relationships with healthcare professionals and referral sources
• Strong organizational and time management skills
• Proficiency in Microsoft Office Suite
