Hospice Intake Nurse

Responsibilities

• Serve as a primary contact for hospice referral sources, outside agencies, healthcare professionals, and field staff.
• Manage incoming calls, providing information to patients, families, and referral sources to facilitate access to services.
• Communicate arrangements for care initiation with patients, clients, and families.
• Conduct intake screenings, gather and analyze required information for preliminary admission decisions.
• Log referrals, prepare reports, respond to inquiries, and promote hospice services to prospective patients.
• Receive verbal orders from physicians where legally permitted to initiate care/services.
• Input customer data into management systems and provide telephonic and on-call support as needed.

Requirements

• Graduate of an accredited nursing program.
• Current RN licensure in the applicable state.
• At least one year of RN experience; hospice or home health preferred.
• Strong communication, organizational, and interpersonal skills.
