About the Role
This position involves acting as a primary contact for hospice referral sources, coordinating incoming calls, and providing essential information to facilitate patient access to care. The RN will communicate with patients, families, and healthcare professionals to arrange services, complete intake screenings, and make preliminary admission decisions.
Responsibilities
- Act as liaison with referral sources and external agencies
- Coordinate and facilitate patient intake and service initiation
- Document and analyze intake information for admission decisions
- Promote hospice services and respond to inquiries
- Handle verbal physician orders when legally permitted
- Maintain accurate records within management systems
- Log referrals and prepare reports
- Provide telephonic support as needed
Requirements
- Graduate of an accredited nursing school
- Current RN licensure in Michigan
- At least one year of RN experience
- Preferred: hospice or home health care experience
