Hospice Admissions Nurse

Responsibilities

• Conduct in-home hospice admission assessments and start-of-care visits.
• Evaluate patient eligibility and educate patients and families about hospice services.
• Develop initial care plans tailored to patient needs.
• Coordinate care with physicians and the interdisciplinary team.
• Support elderly and chronically ill patients in remaining safely at home.
• Utilize strong clinical judgment, compassionate communication, and accurate EMR documentation.

Qualifications & Skills

• Registered Nurse (RN) license.
• Experience in hospice, palliative, or home health care preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently in a field-based setting.
• Compassionate attitude focused on patient-centered care.
