About the role
We are actively seeking a committed and dedicated Intake Coordinator to join our hospice division. In this pivotal role, you will coordinate hospice services to ensure the provision of optimal care through timely and accurate communication and documentation. The successful candidate will embody Bloom's standards of quality and compassion, continually ensuring alignment with our commitment to superior patient care.
Responsibilities
- Review and assess patient medical records and information to determine eligibility for hospice care
- Communicate with healthcare providers, physicians, and other involved parties to gather necessary information
- Interact with patients, their families, and healthcare professionals to explain hospice services and answer questions
- Provide emotional support and guidance to families during the decision-making process of choosing hospice care
- Ensure accurate and thorough documentation of patient information, medical history, and other relevant details
- Manage intake paperwork and obtain necessary consents for hospice care
- Collaborate with other members of the hospice team, including nurses, social workers, and chaplains, to coordinate the admission process
- Coordinate the logistics of transitioning patients from other healthcare settings to hospice care
- Educate patients and their families about the hospice philosophy, services, and what to expect during their time in hospice care
- Ensure compliance with regulatory requirements and documentation standards related to hospice admissions
- Address crisis situations or urgent needs that may arise during the intake process
- Conduct follow-up assessments to ensure the ongoing appropriateness of hospice care for the patient
- Manage referrals from healthcare professionals and agencies, ensuring a smooth transition into hospice care
- Participate in ongoing training and education to stay current on hospice care practices and regulations
Requirements
- Demonstrated customer service skills with at least 1 year of experience
- Proficiency in medical terminology acquired through a minimum of 1 year of experience
- Advanced computer skills, with proficiency in navigating EHR systems and the Microsoft Suite
- Bachelor's degree preferred
Schedule
Tuesday to Saturday, 8am - 5pm
Pay
The pay range for this role in the state of Colorado typically falls between $24.00 - $27.00 hourly, with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
