Schedule
- Full-time 0.9 FTE (36 hours/week)
- Day Shift: Monday–Friday, 8:00 am–4:30 pm
- Includes weekend rotation (every 12th weekend) and holiday rotation (3 holidays/year)
Pay
- Starting pay: $18.50/hour + additional pay based on experience
- Sign-on bonus: $3,000
- Mileage reimbursement
About the role
Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. As a CNA Homecare (HC), you will provide patient- and family-centered care consistent with Agrace’s philosophy, goals, standards, regulations, and policies.
Responsibilities
- Assist patients with activities of daily living:
- Ambulation, changing position, and moving to/from sitting and standing
- Bathroom functions, including toileting and incontinence care
- Bathing, showering, dressing, oral care, and grooming
- Eating and drinking
- Post-mortem care
- Obtaining vital signs and assisting with treatments
- Provide homemaker assistance:
- Light housekeeping: changing linens, tidying rooms, laundry, and trash removal
- Ordering and delivering patient supplies
- Preparing simple meals
- Communication:
- Read, understand, and follow the plan of care
- Document cares and changes in patient condition in the electronic health record
- Report significant changes in condition or unusual incidents to the Interdisciplinary Team
- Reinforce patient, family, and facility partner education
- Miscellaneous:
- Participate in meetings to plan and evaluate patient/family services
- Precept new staff and students
- Delegate tasks to volunteers as appropriate
Requirements
- High school diploma or GED, or equivalent combination of education and work experience
- Graduation from an accredited Certified Nursing Assistant program
- Current, unrestricted Nursing Assistant Certification in the state of employment
- One-year personal care experience, hospice, or home health (preferred)
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Basic Microsoft Office skills
- Experience using electronic health records
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
