Hospice & Home Care Liaison

Responsibilities
- Analyze the potential of the company’s success to determine target markets.
- Visit doctor offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities, and other possible sources of referrals to present agency credentials and obtain patient referrals.
- Analyze the company’s organization to determine its strengths and weaknesses.
- Analyze past and current marketing data.
- Analyze patient/company relationships.
- Develop a marketing plan, identify priorities, and set a reasonable timeline.
- Implement the marketing plan staying within the established timeline.
- Review and evaluate the analysis and plan on an established basis.
- Provide coordination of referrals through all Dover Health services.
- Attend IDG meetings, as appropriate.
- Serve as a first contact from clinical referral sources.
- Provide initial informational meetings with patients and/or families.
- Aid office staff as needed with obtaining signatures and updates.
- Participate in community outreach and education.
- Participate in discharge planning, as needed.
- Assess patient’s/family members’ ability to cope with the patient’s dying.
- Function as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient’s health problems.
- Help patients utilize the resources available to them and their families and the community.
Requirements
- At least 1 year experience in home health or hospice sales/marketing.
- Clinical license and experience preferred.
- Valid driver’s license with an insured automobile in good working order, and a clear driving record.
- Willingness to travel locally and occasionally overnight.
- The ability to communicate well, both verbally and in writing.
- The ability to meet all physical demands of the job, including lifting up to 50 lbs. independently and up to 200 lbs. with assistance.
- The ability to work well with others and take direction from management, as well as take initiative.
- The ability to be self-motivated to accomplish identified goals with a strong sense of accountability for results.
Qualifications
- Current and valid driver’s license, reliable transportation, and current liability auto insurance is required.
- The ability to perform the essential functions of the job with or without reasonable accommodation.
