Job Summary

Primary function is to provide healthcare and related services to the patient in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor.

Responsibilities

  • Promote hospice mission, goals and philosophy.
  • Demonstrate commitment, professional growth and competency.
  • Demonstrate compliance with policies, procedures, regulations defining scope of practice.
  • Promote security and safety of Protected Health Information.
  • Participate in programs and on committees as requested.
  • Participate in ongoing education as required.
  • Provide services according to the Hospice Aide Care Plan.
  • Coordinate with RN-develop/update plan of care to meet the needs.
  • Attend IDG meetings, as requested.
  • Document services and submit timely.
  • Other duties as assigned.
  • Report changes to the RN.

Qualifications

  • State Driver's License.
  • At least one year preferred, as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
  • Strong communication and interpersonal skills.
  • Sensitivity to issues of loss and grief.
  • Ability to deal effectively with the demands of the job and population serviced.
  • Ability to work without consistent direct supervision.
  • Reliable transportation.
  • Bilingual (English/Spanish) preferred.
  • CPR certified.
  • TB health screen or TB test/CXR according to policy.
  • Hepatitis B Vaccination Consent/Declination.
