About the role
In this field-based role, you will visit patients to conduct physical and psychosocial assessments, document visit details electronically, and develop nursing care plans tailored to patient needs. Visits occur in both community and facility settings. As a Hospice Field Case Manager, you'll make care recommendations and coordinate services.
Responsibilities
- Work one weekend per month
- Provide backup On Call support 2–3 times monthly (car and escort services available for On Call shifts)
Qualifications
- Associates or Diploma in Nursing; BSN preferred
- 2+ years' experience in med/surg, community health, or oncology nursing preferred
- NY State RN license
- NY State Driver’s license
