About the role
Responsible for providing compassionate care to patients and families in their homes. Works closely with interdisciplinary teams to coordinate care plans.
Responsibilities
- Assess patient needs and develop personalized care plans
- Provide direct patient care including medication management, wound care, and symptom management
- Collaborate with healthcare providers to ensure continuity of care
- Facilitate communication between patients/families and healthcare providers
- Document all interactions and care provided accurately and timely
Requirements
- Bachelor's degree in Nursing or related field
- Current license to practice as a Registered Nurse in your state
- Experience in hospice or palliative care preferred
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Strong emotional intelligence and empathy
- Proficiency in Microsoft Office Suite
- Knowledge of hospice policies and procedures
Skills
- Effective problem-solving and decision-making skills
- Strong organizational and time-management skills
- Ability to handle sensitive and confidential information
