About the role

Responsible for providing compassionate care to patients and families in their homes. Works closely with interdisciplinary teams to coordinate care plans.

Responsibilities

  • Assess patient needs and develop personalized care plans
  • Provide direct patient care including medication management, wound care, and symptom management
  • Collaborate with healthcare providers to ensure continuity of care
  • Facilitate communication between patients/families and healthcare providers
  • Document all interactions and care provided accurately and timely

Requirements

  • Bachelor's degree in Nursing or related field
  • Current license to practice as a Registered Nurse in your state
  • Experience in hospice or palliative care preferred
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team

Qualifications

  • Strong emotional intelligence and empathy
  • Proficiency in Microsoft Office Suite
  • Knowledge of hospice policies and procedures

Skills

  • Effective problem-solving and decision-making skills
  • Strong organizational and time-management skills
  • Ability to handle sensitive and confidential information
