About the role

We are seeking a dedicated Registered Nurse with experience in hospice/home care settings to join our team. This position involves reviewing patient information, assigning appropriate personnel, and coordinating care with medical directors. The ideal candidate should have two years of management or supervisory experience and a proven ability to work effectively in an interdisciplinary setting.

Responsibilities

  • Reviews case referrals and available patient information to determine hospice/home care needs.
  • Sets up appropriate hospice personnel for cases as needed.
  • Convenes with the medical director to discuss individual eligibility for services.
  • Evaluates cases through various means like home visits, conferences, and record reviews.
  • Communicates with clinicians to improve performance and ensure quality care.
  • Affords assistance to clinicians during operating hours.
  • Collaborates with hospice personnel at case conference meetings to coordinate care and involve interdisciplinary groups.

Qualifications

  • Current RN license to practice in the state.
  • Two years of management or supervisory experience in hospice/home care settings.
  • Excellent observation, judgment, and communication skills.
  • Understanding of hospice philosophy and issues related to death and dying.
  • Knowledge of pain and symptom management.

The pay range for this position is $125,000 - $135,000 annually.
