About the role
Ennoble Care is a mobile primary care, palliative care, and hospice service provider serving New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. Our clinicians visit patients in their homes, offering a continuum of care for those with chronic conditions and limited mobility. We provide programs like remote patient monitoring, behavioral health management, and chronic care management to ensure high-quality care by trusted teams.
Responsibilities
- Provides pastoral/spiritual care to patients and families.
- Visits and ministers with patients, working with the Clinical Director and others to develop a spiritual care plan.
- Conducts spiritual assessments and develops a care plan addressing physical, emotional, and spiritual needs.
- Supports patients and families at bedside, at funerals, and memorials as needed.
- Documents visits according to company and compliance policies.
- Works as a member of the interdisciplinary team.
- Organizes Company-sponsored memorial services as necessary.
- Supports staff emotionally and spiritually, participating in meetings and in-services.
- Communicates with area inter-faith clergy and educates staff on specific religious needs affecting care.
- Facilitates spiritual care, providing sacramental support and pastoral care as requested.
- Assists in educating staff about specific religious needs and issues affecting care.
- Performs other duties as assigned or requested.
- Reports to and is primarily accountable to the Clinical Director.
- Collaborates with the interdisciplinary team to ensure holistic patient care.
- Contributes value-added activities enhancing client, patient, family, and staff well-being.
Requirements
- Master’s of Divinity Degree or equivalent in theological studies.
- At least four units of Clinical Pastoral Education (CPE).
- Board Certification as a Chaplain through the Association for Clinical Pastoral Education (ACPE) or the College of Pastoral Supervision and Psychotherapy (CPSP).
- Excellent communication and pastoral skills.
- A high degree of emotional energy and mental alertness.
- Good organizational and decision-making skills.
- Computer literacy and ability to perform timely charting using the Company’s EMR System.
- Licensure through the CPSP or ACPE.
- A minimum of two years of pastoral experience in a healthcare setting, with hospice experience preferred.
- Demonstrable empathy, compassion, sensitivity, and ability to support patients and families from various religious backgrounds.
- Strong interpersonal skills, ability to work independently, and hold accountability.
- Ecclesiastical endorsement or Ordination in a Religious Setting is desirable.
