Hospice Chaplain
Summary
The Hospice Chaplain is responsible for assisting patients and their families in meeting spiritual and emotional needs during the patient's illnesses. Supports staff members during times of stress and/or crisis. Works closely with volunteer chaplains and area churches.
Qualification Requirements
- Master of Divinity or Master of Religious Education from an accredited seminary preferred.
- Bachelor's in a ministry related field required.
- Three to five years of pastoral care experience preferred.
- Ordination to Pastoral Ministry and denominational endorsement preferred.
- Certification in Chaplaincy, chaplain experience or completion of at least one unit of Clinical Pastoral education or willingness to complete after hire preferred.
- Knowledge of grief process
- Public relations/speaking skills
- Knowledge of hospice philosophy, dying process, and bereavement and spirituality
Essential Functions
- Abides by and supports 3HC's Compliance Program and Code of Ethics.
- Creates positive experiences for internal and external customers that will meet their expectations.
- Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
- Provides spiritual/pastoral care needs to patient/family members:
- Ensures needs and support are provided in accordance with on-going wishes and needs of the patients and family members.
- Offers patients and family members of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs, and values.
- Ensures that the spiritual/pastoral assessment of patient are completed as indicated and appropriate, within 5 days of admission for both home hospice patients and inpatient hospice facility patients.
- Works in close collaboration with local clergy whenever desired by patient.
- Provides spiritual support as needed and defined by the patient/family/significant other.
- Demonstrates progress toward unresolved spiritual issues through documentation of spiritual counseling provided in patient's clinical record.
- Conducts religious services of prayer, worship, and rituals for patients/families as appropriate.
- May assist in developing and updating the plan of care as needed.
- Participates in providing Bereavement Care and Support to families, as well as community at large:
- Assists in facilitation of monthly bereavement support groups as needed.
- Provides other spiritual counseling services as determined appropriate by the IDG members.
- Works with congregations to develop pastoral care support for specific patients and for death and dying issues related to spiritual care.
- Works with other professionals, as determined by the hospice to evaluate patient/family response to care.
- Is available to family during the bereavement period as requested.
- Participates in annual memorial services.
- Makes chaplain bereavement visits and assessments as determined appropriate by IDG team.
- Supports staff in personal spiritual care/pastoral and bereavement care issues as needed that may affect their ability to function effectively.
- Demonstrates an interest in personal and professional growth for self and staff:
- Attends and participates in workshops, seminars, webinars to keep abreast of current changes in rules, regulations, relating to job functions and department.
- Does individual reading and research.
- Recommends educational opportunities for staff and encourages participation.
- Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
- Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
