Hospice Certified Nursing Assistant CNA

About the role

Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary teams to coordinate comprehensive care plans.

Responsibilities

• Administer medication and treatments as prescribed
• Assess patient needs and provide ongoing care plans
• Communicate with healthcare providers and family members
• Coordinate with social workers and chaplains

Requirements

• Bachelor's degree in a healthcare-related field
• Current state licensure as a hospice aide or equivalent
• Experience in home health care or related field preferred

Qualifications

• Strong interpersonal skills
• Ability to work independently and as part of a team
• Excellent communication and organizational skills

Skills

• Basic computer literacy
• Knowledge of hospice care protocols
