Responsibilities
- Assesses patient needs and develops care plans
- Administers medications and treatments as prescribed
- Maintains accurate records and documentation
- Communicates regularly with healthcare providers and family members
- Ensures compliance with hospice policies and regulations
Requirements
- Bachelor's degree in a related field
- Current CPR certification
- Experience in home health care or palliative care preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Qualifications
- Valid driver’s license
- Ability to travel to various locations within the service area
