About the role
Hospice Care Planner responsible for evaluating and establishing care plans for patients referred for Hospice services.
Responsibilities
- Facilitate patient admissions from hospital discharge to appropriate Hospice programs.
- Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners.
- Ensure seamless coordination of hospice care tailored to patient and family needs.
Qualifications
- Licensed Registered Nurse with current registration in NYS.
- Bachelor's Degree preferred.
- Minimum of two years' hospice nursing experience.
Skills
- Strong assessment and interviewing skills.
- Excellent written and verbal communication.
- Client‑service focus.
- Advanced critical thinking.
- Ability to work independently.
- Bilingual (Spanish/English) required in some locations.
