Responsibilities

• Provide hospice care consultations to patients and their families
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
• Assist with patient and family education regarding hospice services and resources
• Maintain accurate and timely documentation of all interactions and care provided

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work)
• At least 2 years of experience in a healthcare setting
• Current CPR certification
• Excellent verbal and written communication skills
• Ability to work flexible hours, including evenings and weekends

Qualifications

• Experience working with individuals facing end-of-life issues
• Strong interpersonal and problem-solving skills
• Proficiency in Microsoft Office Suite
