St. Mary's Home Care & Hospice is seeking a Contingent Certified Home Health aide/C.N.A. for hospice. Provide one-to-one, compassionate care and love your job. Responsibilities
- Follows care plan developed by RN.
- Reports client changes in condition to RN, as described on care plan.
- Aids clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
- Aids clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
- Measures and records client’s oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
- Performs range of motion and other simple procedures as an extension of therapy services as assigned.
- Aids clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record.
Requirements
- High School diploma or GED required.
- Completion of CNA 75 hr course.
- Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
- Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
- Mets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references).
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
- Accepts responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis.
- Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
- Current Driver’s license and reliable transportation to and from work site.
