Hospice Admissions RN (PRN)

About the role

Join a compassionate, mission-driven team with Hospice of Marion County, part of Empath Health. We’re looking for a dedicated and empathetic Admissions RN to make a meaningful impact by guiding patients and families through the admissions process—providing care and support in homes, healthcare facilities, and hospitals throughout Marion County.

Schedule

PRN, minimum of 64 hours a month availability.

Requirements

• Hospice experience required.

Qualifications

• Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Valid Florida license as a Registered Nurse.
• Valid Florida State Driver’s License.

Skills

• Ability to advocate for optimal access to the right care at the right time in any care setting.
• Experience completing the admission process based on care program specifics, guidelines, and patient-specific needs.
• Ability to coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital.
• Ability to work directly with community referral sources to educate regarding Empath Health programs and services.
• Compliance with RN On Call scheduling needs and vendor credentialing process.
