Role Overview Supports hospice patients and their families by providing specialized nursing care in a home or facility setting. Responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with an interprofessional team to ensure quality, culturally sensitive, and patient-centered care. Key Responsibilities Assess patient preferences, barriers, and needs, including emotional, spiritual, and cultural factors.Develop, document, and communicate personalized care plans with team and family involvement.Promote respectful, equitable, and empathetic interactions with diverse populations.Evaluate patient condition changes, update care plans, and collaborate with the team and family.Ensure evidence-based practices and safe care delivery in a safety-sensitive environment. Qualifications & Skills RN license and valid driver’s license required.Associate’s degree in nursing or higher preferred; experience less than 2 years required.Experience in hospice or admissions is preferred.Strong communication, critical thinking, and culturally competent care skills.
