Admissions Nurse/Job Description Clinical Job Functions: Maintain a valid nursing licenseComplete admission and/or collaborate to pass admission to RNCMEducate clinical care team on community/referral source protocolsAttend and complete all required clinical trainings/in-servicesProvide clinical education and/or in-services for referral sources/Bloom staffParticipate in clinical meetings such as IDT when necessaryAttend patient care conferences when necessaryProvides effective patient/family/caregiver teaching as necessaryCreates timely and accurate documentationNew referral assessments at hospitals/ SNF/ RehabsCollaborates with Ambassador on new referrals and processing of referralsAssists community with transfer referralsProcess referrals in conjunction with Intake/Scheduling:Clinically assess referral paperworkClinical collaboration with PCP regarding referralComplete EOB/informational with family/patient/POAObtain consent to admissionProvides Chronic Care Management Palliative care visits in qualifying communities Admissions Nurse Job Functions: Promotes Bloom services and programsSupport Practice Ambassador in building and maintaining referral source relationshipsSchedules and attends sales meetings with referral sourcesAssists with referral source complaints/needsAssist community during a state auditResponsible for building and maintaining relationships, protocols, and expectations between Bloom HBPC Providers and Bloom Hospice staff Qualifications: Accredited nursing school graduateValid CO RN licenseAt least one year in medical, surgical, or critical care nursingStrong clinical assessment capabilityExcellent communication and interpersonal skillsEffective problem-solving skillsDetail-oriented with strong time managementComputer proficiency Additional Qualifications Preferred: Experience in hospice or oncologyUnderstanding of death/dying and bereavement processesCHPN certification Why Bloom? The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $95,000 - $105,000 with the potential for performance-based bonuses and other benefits.
