Homecare RN Case Manager (Mon-Fri, 8am-5pm)

Description

This position demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN Case Manager is responsible for developing and implementing a plan of care in accordance with the patient’s expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care.

• Scheduling patients
• Checking vitals
• Medicine reconciliation
• Documentation for each visit (assessments, interventions, and updates)
• Developing and coordinating plan of care
• Communicating with Nurse Practitioner
• Average of 4 patient visits a day
• Supervising CNA services provided to assigned patients
• Making calls to family to explain patient changes
• Serving as an active member of the interdisciplinary group (IDG) and attending IDG meetings regularly

Qualifications

• Required: Associates or better in Nursing.
• PREFERRED: Bachelors or better in Nursing.
• Required: 1 year of clinical nursing experience.
• Required: RN license.
• PREFERRED: CHPN certification.
