Homecare Assistant Clinical Manager

Responsibilities

• Aid in the daily operations of the Homecare team
• Coordinate patient care and services across disciplines
• Serve as a clinical resource for field staff
• Monitor quality of care and patient satisfaction
• Collaborate with physicians and interdisciplinary team members
• Review clinical documentation and support regulatory compliance
• Assist with staff supervision, productivity, and performance management
• Participate in quality improvement and educational initiatives
• Support patient and family needs while promoting continuity of care

Qualifications

• Current NJ RN License required
• BSN required
• Minimum 2 years of home health/community health experience
• Leadership or supervisory experience preferred
• Knowledge of OASIS and home health regulations
• Strong communication, organizational, and interpersonal skills
• Valid driver's license and reliable transportation

Physical Demands/Working Conditions

• Flexibility to work both indoors and outdoors while using clinical equipment
• Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients

Pay

When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The compensation range for this position is USD $84,000.00 to USD $112,000.00.
