About the role
Responsible for providing home visits to support clients in DeKalb County. Conduct assessments, develop care plans, and coordinate services.
Responsibilities
- Conduct home visits to assess client needs
- Develop and implement care plans
- Coordinate services with other providers
- Maintain accurate records and reports
Requirements
- Bachelor’s degree in related field
- At least 2 years of relevant experience
- Valid driver’s license and reliable transportation
- Ability to work flexible hours including evenings and weekends
