About the role
Healthy Families, Brockton is seeking a Home Visitor to provide support to first-time young parents through the Healthy Families Massachusetts (HFM) program. This position offers a competitive pay range of $20 - $27 per hour, depending on education and experience.
Responsibilities
- Successfully complete all Healthy Families Massachusetts required orientation and trainings
- Practice and promote the mission of HFM and SEMCOA
- Provide consistent, strength-based home visits based on participant needs and service level match
- Complete all required assessments to ensure appropriate service delivery
- Aid in the development of the Family Service Plan and monitor goal progress and achievement
- Plan curriculum according to participant needs, goals, and identified interests
- Maintain all written/electronic records according to HFM policies
- Aid in the development and facilitation of groups and social events for participants
- Attend and actively participate in supervision and staff meetings
- Perform other duties as assigned by the Program Function Manager/Coordinator/Supervisor
Requirements
- Present as courteous and professional at all times
- Demonstrate flexibility and ability to perform multiple tasks
- Show excellent communication skills, including the ability to organize ideas in logical and clear fashion
- Organize and prioritize a demanding work load while adhering to project timelines
- Have knowledge of basic computer skills and software programs, including the ability to learn, use Children’s Trust’s electronic participant record system and HPAO systems and applications
- Know infant, child and adolescent development, parent/child attachment, and maternal-infant health
- Be knowledgeable of area community resources
- Work independently and with others in a cooperative and collaborative manner
- Understand and implement Trauma Informed Care
- Maintain appropriate boundaries and adhere to code of ethics
- Comply with federal and state regulations concerning confidentiality
- Understand and adhere to all applicable policies and procedures of HPAO
- Complete TB screening requirements
- Certify in the HPAO approved de-escalation program
- CPR and First Aid Certification
- Reliable transportation, satisfactory driving record, and a valid Massachusetts Driver’s License for a minimum of three years
- Satisfactory response to a CORI & SORI
Qualifications
- Associate’s Degree in Human Services or related field preferred
- High School Diploma or equivalent required
- Experience working with or providing services to children and families/participants
- Experience with and willingness to work with culturally diverse populations among program’s target population
- Experience in home visiting, curriculum planning and/or group facilitation
