Home Visitor- Asthma Program

About the role

The Home Visitor works within the Community Asthma Prevention Program (CAPP) at Children’s Hospital of Philadelphia (CHOP). CAPP aims to educate families and improve their self-management skills in managing asthma and environmental mitigation to enhance asthma control.

Responsibilities

• Conduct initial assessments of homes to identify asthma triggers and provide asthma education classes in the home.
• Demonstrate and supervise environmental interventions in the home to improve asthma management.
• Aid patients and families in identifying necessary education, information, agency referrals, and advocacy to manage medical and social needs.
• Establish and maintain a caseload of 50 families, contacting them regularly to document the maintenance of interventions.
• Set up appointments with eligible community members for the home visitor program and connect families to community agencies, social service organizations, insurance providers, and other resources.
• Conduct regular follow-up visits in person or virtually to reinforce education and assess the maintenance of the intervention.
• Document all visits, activities, supplies used, and enter data into databases and electronic health records.
• Ensure appropriate use of incentives and intervention supplies, maintain the organization of the storage closet, and document the use of supplies and incentives.
• Assist in training new Home Visitor staff and attend all CAPP staff meetings.

Requirements

• High School Diploma or GED
• Bachelor's Degree in social work, public health, or education (preferred)
• At least three (3) years of experience working in community-based programs, community health (required)
• Experience in educating community residents, particularly Philadelphia residents (preferred)
• Knowledge of local community agencies and organizations
• Strong interpersonal, communication, problem-solving, teaching, and coaching skills
• Ability to work independently and collaboratively with various teams
• Excellent computer skills, including email, Word, and EPIC
• Understanding of equitable access to healthcare
• Ability to navigate smartphones and mobile devices, IOS and Android
• Outstanding customer service and communication skills
• Personal experience with medical complexity is preferred but not required

Qualifications

Pay

SALARY RANGE: $22.61 - $28.26 Hourly
