Home Infusion Clinical Liaison - RN

About the role

We are looking for an experienced and proactive RN / Clinical Liaison to join our care team at BayCare HomeCare. This vital role blends patient-facing responsibilities with clinical coordination and administrative duties, supporting a diverse patient population. The position is based in Largo, FL, with a PRN status and no weekend work or on-call requirements.

Responsibilities

• Facilitates transitions of care from acute and subacute settings to home infusion and educates hospital and community providers on home infusion and enteral therapies.
• Collaborates with providers, case managers, referral sources, and home health agencies to facilitate patient discharge and coordination of care.
• Works with the business development team to gain and maintain market share through the referral intake process.
• Ensures timely communication with all referral sources via phone, electronic platforms, and in-person interactions.
• Provides clear and concise referrals to the pharmacy, meeting all regulatory, payer, and safety requirements.
• Completes preadmission assessments and educates patients and caregivers on pharmacy services.
• Conducts initial infusion and enteral set-up, including patient/caregiver training on equipment and therapy services.
• Initiates formulary changes and recommendations on nutrition routes under the direction of the pharmacy's registered dietitian.
• Maintains knowledge of Medicare and Managed Care regulations and requirements.
• Reviews patient medical records to obtain pertinent medical history and insurance information, communicating this to pharmacy intake.
• Participates in data collection for therapy start and patient tracking, facilitating order transfers via phone, fax, and e-prescribing.
• Ensures timely responses to referral sources, providers, and leadership.
• Monitors and communicates referral source activity, acting as a single point of contact for referral sources, home health, and infusion.

Requirements

• BLS (Basic Life Support) certification.
• Valid Florida Driver’s License and RN (Registered Nurse) License.
• Associate’s or Diploma in Nursing.
• Minimum of 2 years of nursing experience, including 1 year in infusion-related care.
• Business development skills.
• Reliable transportation for travel within assigned territory and attendance at meetings.
• Infusion and enteral administration skills assessment.
• Level II screening through the Florida Care Provider Background Screening Clearinghouse.

Preferred qualifications include computer proficiency in Microsoft Office and Excel, experience in home healthcare specialty pharmacy, and CRNI certification.

Schedule

Shift hours: 8:00 a.m. - 5:00 p.m., Monday - Friday.
