Home Hemodialysis / Peritoneal Dialysis Registered Nurse
About the role
This role involves managing Home Hemodialysis (HHD) and Peritoneal Dialysis (PD) patients, providing clinical care, patient education, and case management.
Responsibilities
- Train patients and caregivers on HHD procedures and self-care
- Manage a caseload of HHD patients, including assessments and care planning
- Monitor treatment progress, conduct home visits, and provide on-call support
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Requirements
- RN license in Utah, and if already licensed in Nevada, that is a plus
- Must have 9 months of post-graduate RN experience for this HHD/PD role
- Valid driver’s license, insurance, and reliable transportation
- ADN required; BSN preferred
- Basic computer skills (MS Word, Outlook)
Preferred Experience
- Dialysis (HHD, PD, chronic, or acute) patient education or teaching background
- Case management or telehealth experience
- CNN or CDN certification

Location
The incoming RN will work primarily in St. George but needs to travel to Hurricane, Mesquite, and Cedar City to speak with patients as needed.
Training and Travel
Majority of training for this role will be done in Utah County with an experienced preceptor. The incoming teammate must be willing to travel to this area during the first 3 months for up to 1 week at a time.
