Home Health Speech Therapist - Full Time

Position Overview The Speech Therapist is responsible for providing high-quality patient care following physician orders. Responsibilities include assessments, administering treatments, care planning, interdisciplinary communication, supply management, emergency response, care coordination, health promotion, and family education. Key Duties Deliver prescribed Speech Therapy services.Develop and document evaluations, treatment plans, and goals aligned with physician directives.Manage patient care plans to meet therapy objectives.Coordinate services with healthcare team, families, and caregivers, including discharge planning.Maintain patient confidentiality and effective communication.Serve as a clinical resource and provide progress reports to physicians and team members. Qualifications & Skills Master’s degree in Speech-Language Pathology from an ASHA-accredited program.Valid and unrestricted SLP license in the practice state.Current CPR certification.Preferred: at least one year of recent experience in an acute care setting.
